Adult Program Specialist (Day Program Direct Care) | 1st Shift | WESTLAKE Essential Functions: The following list is not intended to be comprehensive; it provides a representative summary of the major duties and responsibilities. Assists and advocates on behalf of individuals; maintains respectful and effective communication with individuals and relevant parties. Provides emotional support by developing age-appropriate relationships with individuals. Assists individuals in experiencing all phases of the Pathways Program, including on-site and off-site activities such as transportation, events, learning, re-learning programs, and socialization opportunities. Helps individuals develop and maintain daily living skills to promote independence, including personal hygiene, behavior, dietary habits, and safety. Performs direct support and supervision, including: o Providing transportation for social activities and appointments. o Administering healthcare as needed and following relevant plans and orders. o Upholding individuals' rights and reporting incidents. o Supervising during community outings. o Attending meetings and appointments. o Monitoring and reporting changes in individual well-being. o Understanding emergency procedures. Obtains and maintains certification as a Medication Passer. Attends in-services and continuing education. Maintains confidentiality of all information. Records data on individual progress, health, incidents, and goals. Complies with agency policies, including mandated reporting and individual rights. Contributes to individual plans and documentation. Supports the agency’s mission and values. Adheres to attendance standards. Performs additional duties as assigned. Minimum Qualifications: Must be at least 18 years old. Valid Ohio Driver’s License with acceptable driving record. Reliable transportation and at least two years of driving experience. Proof of High School Diploma or equivalent. Criminal background compliant with regulations. Participation in the agency’s drug-free workplace program. Ability to communicate respectfully with individuals, families, and staff. Ability to handle stressful situations with emotional stability. Ability to complete required training and continuing education. Dependability, good moral character, and effective communication skills. Experience working with individuals with developmental disabilities is preferred. Environmental and Physical Requirements: May be exposed to infectious illnesses and aggressive individuals. Physical exertion including walking, bending, reaching, kneeling, squatting, pushing, pulling, and lifting 50 lbs. or more. Corrected hearing and vision to normal range.
